Facility Information:
- Rate: $125.00 per person
- Facilities will be charged a $35 travel fee if distance exceeds 100 miles.
- To schedule a training session at your facility, you must have a minimum of 5 trainees.
- When scheduling please insert facility name and point of contact name.

Individual Class information:
- Refer to the class schedule/payment section below for available training dates by using the drop down arrow to select a class.
- Please ensure you have selected the correct class before proceeding to the payment page to secure your spot.
- After payment is received, A formal invitation to training will be sent to your registered email address with details.
- No refunds. You may reschedule your training with a 72 hour notice to avoid loss of fee.
- NOC shift trainees please do not work the night before your scheduled training.
- Trainees must be 18 years of age and bring a valid state ID or DL to the training session for verification purposes and entry.
- Trainees will not receive a certificate of completion. You will be added to the verification website as proof of passing the required modules and meeting the necessary criteria. Please refer to CDPHE website link above to review this guideline and other helpful information.
All trainees are required to review the attached study guide prior to the training session.
